Central Centrifugal Cicatricial Alopecia, or CCCA, is a form of scarring hair loss that begins at the crown of the scalp and spreads outward in a roughly circular pattern. It disproportionately affects Black women, and because its early stages can look like ordinary thinning or breakage, it's frequently missed until significant, irreversible follicle damage has already occurred.
The Genetics Behind CCCA
A landmark study published in the New England Journal of Medicine (2019) identified variants in the PADI3 gene, a gene involved in hair shaft formation, in a significant portion of women with CCCA, establishing a genetic component to a condition long attributed mainly to styling practices. This doesn't mean styling habits are irrelevant; genetic susceptibility appears to interact with tension and chemical exposure to determine who develops the condition and how quickly it progresses.
Early Signs to Watch For
Persistent tenderness, itching, or a burning sensation at the crown (not the hairline), hair at the crown that breaks or falls with unusual ease compared to the rest of the scalp, a smooth, shiny appearance to the scalp in a small area at the crown, and gradual, centrifugal (outward-spreading) thinning rather than an even, all-over pattern are the signs most worth acting on early.
Why Early Diagnosis Changes the Outcome
CCCA is a scarring condition, meaning that once a follicle is fully replaced by scar tissue, hair will not regrow from it; no product or treatment can reverse that stage. Caught early, however, the progression can often be slowed or stopped with appropriate dermatological treatment, which is why any of the signs above are worth a prompt visit to a dermatologist rather than a wait-and-see approach.
Reducing Modifiable Risk
While genetics can't be changed, reducing scalp tension from tight styles, spacing out chemical relaxer and heat use, and treating any scalp inflammation promptly are all modifiable factors that appear to influence CCCA risk and progression.
